Rolling over HSA funds to your new Optum HSA

You have the option to rollover previous HSA funds to your new Optum account.  The Archdiocese cannot do this for you as this account is your own personal bank account.

  1. If your account was with Elements, please click here for the appropriate form, complete and return as instructed on the form.
  2. If your account was with TCU, please click here for the appropriate form, complete and return as instructed on the form.
  3. If your HSA was previously with any other HSA bank, please click here for the appropriate form, complete and return as instructed on the form.

For further assistance, you will need to contact Elements, TCU or your HSA bank directly with any questions regarding this rollover process.

Funds:  If your Optum account was opened in time, your first deposit will be on check date January 5, 2018.  If you have a balance in your old HSA account, you can leave the funds in that account and continue to pay from it until you’ve depleted the account or you can roll it over to your new Optum account.  Whichever you prefer to do.  HSA funds are yours to keep – you do not lose out on any funds.

Debit cards:  Optum only mails one debit card even if you’re on a family health plan.  If you need additional cards, please contact Optum directly.  Cards are only printed in the account holder’s name (employee’s name).
