Effective January 1, 2018, you can earn $800 upon completion of the UMR Maternity Management program. To be eligible to enroll, you must be enrolled in the Archdiocese medical plan at the time you become pregnant. To be eligible for the incentive payout, you must be a current/active employee and enrolled in the Archdiocese medical plan at the time the incentive is to be paid out.
Please contact your Plan Advisor at the phone number on your ID card to self-enroll in the plan. Or you’ll be identified and invited to participate by UMR once they identify you as pregnant through a clinical health risk assessment, utilization review or other program referrals.
