Live Well Wellness Programs
How the program works
Beginning with 2018, the wellness program will be run on an annual calendar basis. This helps to promote your relationship with your physician by ensuring you see him/her each year thus allowing you to work together as a team to manage your own overall health and help reach personal health goals.
The CHRA (clinical health risk assessment) and biometric screening are the 2 activities that must be complete by the end of the year in order to receive an incentive. Completing the CHRA allows for you to learn more about your current health status, including personal strengths and opportunities for improvement. Biometric screening results, when combined with the CHRA, provide a more complete picture of your overall health.
The wellness program starts over on January 1 of the next plan year.

Disease Management Programs
UMR offers expert resources and one-one-one personal health coaching to help those with chronic conditions. To be eligible to enroll in this program, the employee and their spouse must be enrolled in the Archdiocese medical plan as of January 1, 2018. An $800 incentive will be paid to the employee’s HSA/HRA upon completion of the program. To be eligible for the incentive payout, the employee/their spouse must also 1) be a current/active employee and 2) the employee/their spouse must be currently enrolled in the Archdiocese medical plan at the time the incentive is to be paid out.
- Heart failure
- Heart disease
- Depression (when occurring w/one of the other listed conditions)
You will be invited by UMR to join one of these programs. UMR will extend an invitation to you based on any claims that may be associated with one of the above mentioned conditions. Maternity Management Program
Employees or their spouses can earn $800 upon completion of the UMR Maternity Management program. To be eligible to enroll, the employee/their spouse must be enrolled in the Archdiocese medical plan at the time the employee/their spouse becomes pregnant. To be eligible for the incentive payout, the employee/their spouse must also 1) be a current/active employee and 2) the employee/their spouse must be enrolled in the Archdiocese medical plan at the time the incentive is to be paid out.

Or you/your spouse may also be identified and invited to participate by UMR once you/your spouse have been identified as eligible through a clinical health risk assessment, utilization review or other program referrals.
